New information released about “suspicious” deaths at Landmark Recovery

The St. Joseph County Police Department has released more information regarding the death investigations underway at Landmark Recovery:

On July 3rd at approximately 12:30 p.m., the St. Joseph County Police and Penn Fire Department were dispatched to 60257 Bodnar Blvd. in Mishawaka for a report of a deceased male. County Police arrived at Praxis to find Seth M. Jones, age 19 of Montpelier, Indiana deceased. The St. Joseph County Corner was also called out and an initial investigation into the death was begun by the County Police and the Coroner.

On July 4th at approximately 7:00 a.m., County Police and Penn Fire were again dispatched to 60257 Bodnar Blvd. for a report of a second death. County Police arrived at Praxis to find William Breda, age 46 of Griffith, Indiana deceased. The two deceased males were found in rooms across the hall from each other. The St. Joseph County Coroner again was called out to the death scene and an initial investigation into the second death was begun by both the County Police and the Coroner.

That same day, July 4th, at approximately 1:00 p.m., County Police and Penn Fire were again dispatched to 60257 Bodnar for a report of an overdose. A male, age 22, was revived from a suspected overdose after being administered 3 doses of Narcan by Fire and Police. The male was then transported by ambulance to a local hospital for further treatment.

St. Joseph County Police and County Coroner are still awaiting toxicology and final autopsy
reports as well as awaiting turnover of security video and records by Landmark management. All three of these cases are suspected of involving some type of drug overdose; however, aspects of the incident are suspicious, and no conclusions or determinations have been made at this time.

Lastly, at approximately 9:00 p.m. on Sunday, July 9th, Penn Fire and EMS were dispatched to 60257 Bodnar Blvd. for a report of the smoke alarm. Upon arrival at Praxis, Fire and EMS personnel discovered the body of a 28-year-old deceased male. County Police and the County Coroner were then called to the scene to investigate this third death. Initial indications are that the male committed suicide. However, again County Police and Coroner are still awaiting toxicology and final autopsy reports as well as awaiting turnover of security video and records by Landmark management that are needed to make a final determination into the cause of death.

All four incidents currently remain active and ongoing investigations.

ORIGINAL STORY: St. Joseph County Police are investigating some recent deaths that have occurred at Landmark Recovery.

Officers were called to the facility at 60257 Bodnar Boulevard, in Mishawaka, last week, to investigate three patient deaths.

The County Corner and County Police say two of the deaths, which occurred on July 3 and July 4, are suspicious.

They are also looking into an apparent suicide death that happened on Sunday evening, July 9.

Authorities have not released the names of any of the people who died.

Landmark Recovery issued the following statement on Monday night, July 10:

“At Landmark Recovery, patient care and safety are our top priorities. Addiction is a devastating disease that impacts every socioeconomic level and because of that, we have made it our mission to not only serve those with commercial health insurance but to provide high-quality care for those with Medicaid, a population that has very few choices or support options for addiction recovery. We are saddened to share that we had 3 patients pass away at our Praxis facility in South Bend, Indiana. At this time, we cannot share any further patient information due to HIPAA and 42 CFR Part 2 regulations. However, we are working closely with authorities and conducting an internal investigation into these events.”
