A Middlebury concrete company faces fines after inspection, employee death

A concrete mixing company in Middlebury is facing safety fines after the death of a worker.

45 year-old Jason Andresen of Goshen died at a construction site in Middlebury on December 11, 2020.  His body was found in a concrete mixing truck and his death was ruled accidental due to blunt force trauma, according to The Elkhart Truth.

His death prompted an inspection by the Indiana Department of Labor.

Ozinga Ready Mix Concrete of Middlebury were informed of four serious safety violations, most of them related to employees crawling into the drums of cement trucks to clean them. Each order carries a proposed penalty of $7000.
