Troubled Praxis Landmark Recovery facility in Mishawaka reportedly has license revoked

After an investigation due to the death of two people from overdoses and one from suicide, the Troubled Praxis Landmark Recovery facility in Mishawaka has reportedly had its license revoked.

A lawsuit was filed by former patients earlier this week, and there have also been reports of other overdoses and problems at the facility.

The revocation of the Mishawaka facility’s license also impacts the Carmel and Fort Wayne company locations.
