A Middlebury factory has been fined after an employee died in April.

Winnebago of Middlebury was fined a total of $10,000 by the Indiana Occupational Health and Safety Administration (IOSHA).

This is for violations related to the death of 54-year-old Ricky Schlabach. He died in April due to blunt force trauma after being hit by a forklift. A $5,000 fine was issued for installing plexiglass windshields on the forklift without manufacturer approval. The other $5,000 fine was for operating the forklift at an unsafe speed.

The company has 15 days to either pay the fine or appeal it. The violations must be corrected by August 22.
