Two administrators no longer employed at Landmark rehab center

Two of the directors at Landmark Recovery are no longer at the facility amidst an investigation into suspicious deaths.

The St. Joseph County police continue their investigation into two suspicious deaths and an overdose, all of which happened within 24 hours earlier this month.

Praxis, who operate Landmark, says The Joint Commission (TJC) visited the Landmark facility on Friday, July 14, to conduct a detailed survey. Praxis says the full report is not yet available, but the preliminary findings “indicate that our care model is sound,” and they say they look forward to sharing more in the future.

Landmark Recovery says they have separated employment with the Executive Director and the Director of Nursing at the facility, just south of Mishawaka. The announcement came one day after Sheriff William Redman called for the revocation of the rehab center’s license. Redman says the facility has also failed to cooperate with a number of criminal investigations.

Praxis says top leadership will be coming in to supervise the facility, which has stopped taking new patients until further notice. Senior management members including COO AJ Henry, CMO Dr. Jason Kirby, Patient Engagement Director Patrick Kullman, Quality Assurance VP Jessica Tate, and Nursing VP Troy Sparks, will supervise the facility around the clock. The parent company says this ensures their patients stay safe during this time.

Landmark has also voluntarily stopped admitting new patients to the Praxis facility until further notice. They say they will reopen for admissions once they confirm that “all staff meet our high standards.”

This was long time coming for Landmark. I’m am former employee. They never did proper checks Night shift would find somewhere to sleep during thier shift. just a bad place.
