An Indianapolis home health agency is accused of taking more than $10.9 million in Medicaid payments by billing for caregiver hours that prosecutors say never happened.

The case involves Senior Home Care Agency, which launched in 2023 to provide Medicaid waiver services. Prosecutors accuse Alexander Byrnes, 39, of Indianapolis, of controlling the company while Faith Casas, 26, of Carmel, presented herself as the owner to get the agency approved as a Medicaid provider.

The Indiana Attorney General’s Medicaid Fraud Control Unit investigated the case after a complaint was filed in 2025. Investigators reviewed bank records, emails, tax documents and employee interviews.

Court documents say the agency submitted false caregiver hours and received Medicaid reimbursements from late 2023 through mid-2025. Investigators also say Byrnes used company funds for personal expenses, including luxury travel, designer purchases, hotels and entertainment.

The case also involves Cortez Crook and LaShawn Wright, who prosecutors accuse of submitting false caregiver hours.

Byrnes was arrested July 21 and faces multiple felony fraud charges. Casas has an active warrant. The defendants are presumed innocent unless proven guilty in court.
