Review completed of deadly K-Fex fire in New Carlisle

Officials say a number of factors led to the death of a New Carlisle firefighter more than four years ago.

The National Institute of Occupational Safety and Health has completed a review of the fire at K-Fex Excavating that took the life of Assistant Fire Chief Jamie Middlebrook, according to 95.3 MNC’s reporting partners at ABC 57.

He was trapped when the roof collapsed in the building.

The report concludes incident management, communications and lack of a safety officer on site were contributing factors. Several fire departments battled the fire that night, ABC 57 reported.
